Please download the membership application form and complete with the full name and signatures of your proposer and seconder who must be APS members. If you do not know any members who may be able to act as a proposer or seconder please contact the APS Director in your state.

[Separate membership is required for the International Association for the Study of Pain (IASP) which entitles you to their journal Pain and newsletters. An applicant for membership must be sponsored by two regular members of IASP]
